Angina/chest pain

My dad has been diagnosed with CHF (congestive heart failure) after a blood clot came loose from his leg and went into his heart causing quite a bit of damage. This happened quite some time ago but now he has developed the angina. He seems to wake up most mornings with chest pain that he describes as like having a burp stuck. He takes his Nitro which makes his arms ache, he sleeps for a couple of hours and then seems to be fine. When we asked the doctor he said, "yeah, most people with CHF have a problem in the morning." Wow, what a help that was - ha ha. So we tried changing the time he takes is blood pressure medication to dinner time instead of first thing in the morning thinking that maybe his medication was wearing off. He was good that way for about a week and a half and then it started over again. He doesn't seem to have any other symptoms such as sweating, or trouble breathing or anything else, just the pain and it does not radiate. Has anyone had an experience with this and gotten any information to help with this morning problem that seems to happen with a lot of CHF patients. He is fine the rest of the day. Occasionally he has had pain in the evening but very, very rarely.

Any ideas? His bed is elevated and he does have to frequently urinate throughout the night (prostate cancer, cured).
